What Is BBS Form 37A-638?
BBS Form 37A-638 is the official “Associate Professional Clinical Counselor (APCC) Weekly Log of Experience Hours” issued by the California Board of Behavioral Sciences. The current version was revised 08/2022. It is the “Option 1” format — the streamlined column layout — and its PDF file name is lpcc_wkylog_37a-638_option1.pdf.
You fill it out week by week to record your supervised counseling experience. Each form has rows for up to 10 “Week of:” entries, a “Total Hours” row at the bottom, and a supervisor signature area for each week. It is the APCC counterpart of the equivalent LMFT weekly log, Form 37A-525.
Importantly, you do not submit Form 37A-638 to the Board unless it is specifically requested. You keep your completed, signed logs and submit them with your Application for LPCC Licensure at the end of your associate period — they are the supporting detail behind your LPCC hours.
Keep the originals
You are responsible for retaining signed copies of all your weekly logs — your supervisor does not keep them. Store them somewhere safe, back them up digitally, and be ready to produce them if the BBS asks. Hours you cannot document may not count.
Who Needs to Use Form 37A-638?
Form 37A-638 is for Associate Professional Clinical Counselors (APCCs) accruing supervised experience toward LPCC licensure in California. Filling Out the Header
Before you log a single hour, complete the identifying information at the top of the form. Every field matters — an incomplete header is one of the easiest things for the Board to flag.

Use a separate log for each supervisor and for each work setting. If you have two supervisors at one site, that is two forms. If you have one supervisor at two sites, that is also two forms. Never blend supervisors or sites on a single log.
The Columns: A, B, B1, B2, and C
Each weekly row has columns for direct counseling, non-clinical experience, two supervision subcategories, and a weekly total. Knowing which columns are primary totals and which are informational subcategories is the key to filling the form correctly.

Column A — Direct Counseling. Face-to-face direct counseling experience with individuals, groups, couples, or families. This is the clinical work that counts toward your minimum 1,750 direct hours.
Column B — Non-Clinical Experience. Everything that supports the clinical work: supervision, psychological testing, writing clinical reports, writing progress or process notes, client-centered advocacy, and workshops, seminars, training sessions, or conferences.
Columns B1 and B2 — Supervision subcategories. Of the hours you put in Column B, B1 records how many were individual or triadic supervision and B2 records how many were group supervision. They are slices of Column B, recorded so the Board can see your supervision mix. They are not extra hours.
Column C — Weekly total. Column C = A + B. That is it. Because B1 and B2 are already inside B, you never add them on top — doing so would double-count your supervision hours.
The Column C Formula: A + B = C
This is the single most common error on Form 37A-638, so it is worth being explicit:
C = A + B
B1 and B2 are NEVER added to C — they are already inside B
Worked example. Suppose in one week you logged:
Column C = 16 + 5 = 21. Not 23.5. The 1 hour in B1 and the 1.5 hours in B2 are already part of the 5 hours in Column B. If you added everything (16 + 5 + 1 + 1.5), you would be double-counting 2.5 hours of supervision.
The “Total Hours” row at the bottom of the form adds up each column down the page — so the total in the C column should equal the sum of the A and B column totals. If it does not, you have an arithmetic error somewhere in the rows.
How Many Logs Do You Need?
The rule is one Form 37A-638 per supervisor per work setting. Here are the situations that trip people up:
2 supervisors, 1 site
Two logs. Each supervisor signs only for the hours they actually supervised. Never mix supervisors on one log.
1 supervisor, 2 sites
Two logs. Each form is tied to a specific work setting address, even if the same supervisor oversees you at both.
New calendar year
Start a fresh form each January 1. The Year field in the header must match the 'Week of:' dates in the rows.
More than 10 weeks with one supervisor at one site
Start a continuation form. Each log holds up to 10 'Week of:' entries; carry over to a new sheet once you fill it.

Supervisor Signatures
Form 37A-638 has a supervisor signature area for each week. Your supervisor signs off on the hours for that week — this is part of why the form is filled out and signed on a rolling basis rather than backfilled months later.
Have your supervisor sign each week as it is completed. Batch-signing a whole quarter at once creates compliance risk and makes the log harder to defend in an audit. If your supervisor leaves your agency, get all outstanding signatures before they depart, keep the partially completed log, and start a new log with your new supervisor.

The Weekly Cap on Experience
The cap is not printed on Form 37A-638 itself, but it governs the hours you record: under BBS rules, an APCC may count a maximum of 40 hours of experience in any 7 consecutive days across all settings combined, and a maximum of 6 hours of supervision credit per week.
If you work at two sites and log 25 hours at one and 20 at the other in the same week, the combined total is 45 — the BBS counts 40 and the extra 5 are lost. The same logic applies to supervision: anything past 6 hours of supervision in a week does not earn additional credit.

In short: the 37A-675 is the document the Board reviews; the 37A-638 logs are the detailed backup that supports it. You need both — keep your weekly logs current so that, when it is time to fill out the 37A-675, the numbers are already there.
