How to Become a Therapist in North Carolina

North Carolina routes every pre-licensed clinician through one of three separate boards, each running its own hours, its own supervision math and its own associate license. This guide compares all three tracks, then walks each one from degree to full license with a cite at every number. For the full hour and supervision breakdown once you know your track, see North Carolina LCMHC requirements and North Carolina LCSW requirements.

Overview

Three Boards, Three Licensed Paths

North Carolina licenses psychotherapy and counseling practice through three separate, independent boards, not one central agency. The North Carolina Board of Licensed Clinical Mental Health Counselors (NCBLCMHC) regulates the Licensed Clinical Mental Health Counselor Associate (LCMHCA) and Licensed Clinical Mental Health Counselor (LCMHC) credentials, along with the LCMHCS supervisor credential, under G.S. 90-329 et seq. and 21 NCAC 53. The North Carolina Social Work Certification and Licensure Board (NCSWCLB) regulates the Licensed Clinical Social Worker Associate (LCSWA) and Licensed Clinical Social Worker (LCSW) credentials under G.S. Chapter 90B and 21 NCAC 63. The North Carolina Marriage and Family Therapy Licensure Board (NCMFTLB) regulates the Licensed Marriage and Family Therapy Associate (LMFTA) and Licensed Marriage and Family Therapist (LMFT) credentials under G.S. 90-270.45 et seq. and 21 NCAC 31.

All three pre-license credentials are licenses in their own right, issued before supervised practice begins. The counselor credential carried a different name until recently: Senate Bill 537, S.L. 2019-240, renamed the Licensed Professional Counselor Associate and Licensed Professional Counselor Supervisor titles, effective January 1, 2020, to the LCMHCA and LCMHCS titles used today. An old "LPC" reference in North Carolina points to today's LCMHC.

Counseling Track

LCMHCA to Licensed Clinical Mental Health Counselor

The LCMHCA application requires 60 semester hours or 90 quarter hours of graduate training in counseling or a related field from a CACREP-accredited program, plus three semester hours each in three named coursework areas, for applications filed on or after July 1, 2025 (21 NCAC 53 .0701(2), (5)); the internship must take place entirely in a clinical mental health setting under that same effective date (21 NCAC 53 .0206(c)). Before the license issues, the candidate sits one of the NCE, NCMHCE or CRC exams, required for both the LCMHCA and the LCMHC (21 NCAC 53 .0305(a)), and a no-fail jurisprudence exam, required "for licensure application at each level ... and for each consecutive renewal period," taken within six months of the application or the license's expiration (21 NCAC 53 .0305(c)).

An LCMHCA "shall not practice unless" a supervision contract naming the supervisor, the modality, the frequency and the practice site has been submitted and approved (21 NCAC 53 .0702(1)(a)-(e); Supervision Contract form). The Supervision Contract form, revised 12/18/2025, states: "Hours accrued without a Board-approved contract on file will not be accepted." A separate contract is filed for each work setting and for each supervisor (21 NCAC 53 .0702(1), (2)).

Once supervision begins, an LCMHCA works toward a minimum of 3,000 hours of supervised clinical mental health practice, including 2,000 hours of direct counseling experience, gained at no more than 40 hours a week (G.S. 90-336(c)(2); 21 NCAC 53 .0205(a)); the rule states no cap on the remaining, indirect hours. Supervision runs at a minimum of one hour of individual or two hours of group clinical supervision per 40 hours of practice, and those supervision hours do not count toward the 40 hours of practice themselves (21 NCAC 53 .0208; Board LCMHCA page). By the time the 3,000 hours are complete, the LCMHCA must have documented at least 100 hours of clinical supervision, no less than three-quarters of it individual (21 NCAC 53 .0205(a)). Individual supervision covers one or two supervisees at a time; group supervision runs up to 12 supervisees (21 NCAC 53 .0210; .0211). Live video supervision counts as long as it is synchronous, with verbal and visual interaction throughout the session (21 NCAC 53 .0212).

The supervisor files a Quarterly Supervision Report each quarter supervision occurs, though "the Board does not require the submission of hours" on that form (Board LCMHCA page). Hours are recorded once, on the Final Supervision Report, which the supervisor files "upon termination or completion of supervision" (the Quarterly Supervision Report form puts it at within two weeks of termination), and which the Board will not accept directly from the LCMHCA (21 NCAC 53 .0702(3), .0801(c); Board LCMHCA page). To apply for the LCMHC, the candidate submits the Verification of Supervised Professional Practice, completed by the qualified clinical supervisor and carrying the indirect, direct, individual and group supervision totals; that form "cannot be submitted by the applicant" (Verification of Supervised Professional Practice, Revised 6/2/2026). An LCMHCA who wants to avoid a lapse in licensure applies for the LCMHC "no earlier than 60 days prior to expiration" of the associate license, or once the hours are complete (21 NCAC 53 .0701).

Social Work Track

LCSWA to Licensed Clinical Social Worker

The LCSWA credential requires an MSW or DSW from a CSWE-accredited program; the Board requires no exam for the LCSWA itself (G.S. 90B-7(f)). The associate license "may not be issued for a period exceeding two years," and the person holding it "must practice under the supervision of a licensed clinical social worker or a Board-approved alternate" (G.S. 90B-7(f)).

Before practicing, an LCSWA files an emergency crisis plan giving "immediate access to at least one licensed mental health professional" for emergency clinical consultation, where "immediate" means within one hour, filed per practice location and resubmitted within seven days of any change (21 NCAC 63 .0210(b)). The LCSWA also files an Employment Verification form covering the employer, the job description and whether the supervisor is on-site or off-site (21 NCAC 63 .0210(c)), and the supervising LCSW signs a Position Statement on Clinical Supervision before practice begins, filed again whenever the supervisor changes (Position Statement on Clinical Supervision, amended 05/01/26).

The work itself is 3,000 clock hours of work or employment "for a fee or salary" in clinical social work practice, with practicum or internship hours excluded, accumulated "over a period of time not less than two years nor more than six consecutive years" (21 NCAC 63 .0211(a)(1)). Supervision is "conducted no less than once every two weeks, with at least one hour of supervision during every 30 hours of experience," for a minimum of 100 hours total, with a maximum of 25 hours of group supervision (21 NCAC 63 .0211(a)(3)). Every six months, the associate and the supervising LCSW file a Six-Month Review documenting individual and group supervision hours and the clinical practice hours attained in that period, with a separate review for each employer (21 NCAC 63 .0210(e); Board LCSWA FAQs).

After two years of documented clinical practice, the applicant, not the supervisor, submits the Clinical Exam Request Form; the supervisor's role on that form is limited to an attestation "that the applicant has completed two years of clinical practice and is qualified to take the ASWB Clinical level examination" (21 NCAC 63 .0209(a), (f)). Once the ASWB Clinical exam is passed and the Six-Month Reviews and hour totals are complete, the associate files the LCSW Short Form Application, a process the Board's FAQ says "may take a minimum of 30 days" (Board LCSWA FAQs).

Marriage and Family Therapy Track

LMFTA to Licensed Marriage and Family Therapist

The LMFT track begins with a master's degree program; the National MFT Examination, administered by the AMFTRB, may be taken "in the last semester of the program" (Board FAQ). The LMFTA application must include "an agreement by at least one supervisor approved by the American Association of Marriage and Family Therapy" (G.S. 90-270.54A(a)(3)); an eligible supervisor holds the AAMFT Approved Supervisor designation or otherwise meets the standards in the AAMFT Approved Supervision Designation: Standards Handbook (21 NCAC 31 .0502(a)); the Board's FAQ names an "AAMFT Approved Supervisor or accepted Supervisory Candidate."

The LMFTA license is "valid for three years from the date of issuance" (G.S. 90-270.54A(b)) and "shall not be renewed," though a one-year extension is available on written petition showing "special circumstances and steady progress," for a $50 fee (G.S. 90-270.54A(c); 21 NCAC 31 .1001(10)). While holding the LMFTA, the therapist files an annual renewal with proof of 20 hours of continuing education each year (21 NCAC 31 .0701(a)) and must disclose to clients that they practice under ongoing supervision, along with the supervisor's name (21 NCAC 31 .0802(b)).

Within that three-year term, the LMFTA works toward at least 1,500 hours of supervised clinical experience, of which no more than 500 may come from a master's degree program and at least 1,000 must come after the degree (G.S. 90-270.54(a)(1)b). At least 500 of the 1,500 hours must be relational hours: therapy with more than one client in the room who share the same treatment plan (21 NCAC 31 .0502(e)). Assessment and psychoeducation hours are each capped at 250 (21 NCAC 31 .0506(b), (c)). Supervision runs a flat 200 hours, concurrent with the 1,500 practice hours, with at least one hour of supervision required every month until the LMFT is issued (21 NCAC 31 .0502(d)). Peer supervision, supervision by a current or former family member, and administrative supervision "by an institutional director or executive" do not count (21 NCAC 31 .0502(c)).

Time Limits

How Long You Can Hold Each Associate License

LCMHCA: no time limit is stated. The LCMHCA "shall renew" while the supervision requirements to become an LCMHC remain incomplete, and the rule does not state a cap on how many times that renewal can happen (21 NCAC 53 .0702(6)).

LCSWA: six years total, across three renewal cycles, from the date the LCSWA license was first issued; supervision and experience hours acquired under an associate license expire on that same six-year clock (G.S. 90B-7(f)).

LMFTA: three years from issuance, plus one one-year extension by petition (G.S. 90-270.54A(b), (c)).

A Board FAQ adds that if the LMFTA term and its extension both run out, the therapist must apply for a new LMFTA license, and "hours accrued during the initial LMFTA will not carry over" into that new license, though the national exam score does not expire (Board FAQ).

Supervision Math

How the Three Boards Count Supervision

The three boards do not share a supervision formula. Counselors work at a rate of one hour of individual or two hours of group clinical supervision per 40 hours of practice (21 NCAC 53 .0208). As arithmetic, not a separate requirement: if every supervision hour were individual, that rate across 3,000 practice hours would come to 75 hours. The rule separately sets a 100-hour floor, with at least three-quarters of it individual (21 NCAC 53 .0205(a)).

Social work runs a tighter clock: supervision no less than once every two weeks, with at least one hour of supervision for every 30 hours of experience (21 NCAC 63 .0211(a)(3)). The Board's own example puts this in practice: a supervisee who accrues 890 hours of clinical practice in six months must document at least 30 hours of supervision in that period (Board LCSWA FAQs).

Marriage and family therapy skips the ratio altogether. The requirement is a flat 200 hours of supervision across the whole 1,500-hour term, with a separate floor of at least one hour every month until the LMFT is issued, regardless of how many practice hours were logged that month (21 NCAC 31 .0502(d)).

Pitfalls

Common Mistakes Across the Three North Carolina Tracks

1

Reading the supervision rate as the total

The counselor rate of one hour individual or two hours group per 40 hours of practice works out to 75 hours over the full 3,000, but the rule separately requires at least 100 hours, no less than three-quarters individual (21 NCAC 53 .0208; .0205(a)).

2

Assuming a co-worker at the same practice can't supervise an LCSWA

The Position Statement bars supervision by the associate's own employee, not by someone who works at the same organization: "a supervisor shall not be considered an employee of the associate licensee if the only compensation received by the supervisor is payment for actual supervisory hours" (Position Statement on Clinical Supervision).

3

Reading "once every two weeks" as "twice a month"

A 14-day interval and a calendar month are not the same schedule. The LCSWA rule uses the two-week interval (21 NCAC 63 .0211(a)(3)).

4

Assuming the LCMHCA has a renewal cap like the LCSWA's six years

No cap is stated in the counselor rule. The LCMHCA renews while hours remain incomplete, with no stated limit on renewals (21 NCAC 53 .0702(6)).

5

Assuming the Quarterly Supervision Report carries hour totals

It does not. "The Board does not require the submission of hours on Quarterly Supervision Reports." Only the Final Supervision Report carries hours (Board LCMHCA page).

6

Assuming the supervisor files the Clinical Exam Request Form

The applicant submits it. The supervisor's role is limited to the attestation printed on the form (21 NCAC 63 .0209(f)).

FAQ

FAQ: How to Become a Therapist in North Carolina

What is an LCMHCA in North Carolina?▾

An LCMHCA, Licensed Clinical Mental Health Counselor Associate, is a person licensed by the North Carolina Board of Licensed Clinical Mental Health Counselors to practice counseling under supervision while completing the hours required for full LCMHC licensure. North Carolina law makes it unlawful for anyone to use the letters "LCMHCA" without holding that license (G.S. 90-330(a)(2); G.S. 90-331).

How do I get a counseling license in North Carolina?▾

A North Carolina counseling license starts with a graduate degree (CACREP-accredited, 60 semester hours, for applications on or after July 1, 2025), the NCE, NCMHCE or CRC exam, and a jurisprudence exam, then an LCMHCA license backed by a Board-approved supervision contract (21 NCAC 53 .0701; .0305(a), (c); .0702(1)). The LCMHCA works toward 3,000 hours of supervised practice, including 2,000 direct hours and at least 100 hours of clinical supervision, before applying for the LCMHC (G.S. 90-336(c)(2); 21 NCAC 53 .0205(a)).

How do I get a social work license in North Carolina?▾

A clinical social work license in North Carolina starts with an MSW or DSW from a CSWE-accredited program and an LCSWA license from the North Carolina Social Work Certification and Licensure Board (G.S. 90B-7(f)). The LCSWA works 3,000 paid clinical hours over two to six years, supervised no less than once every two weeks, then submits a Clinical Exam Request Form after two years of practice, passes the ASWB Clinical exam and files the LCSW Short Form Application (21 NCAC 63 .0211(a)(1), (a)(3); .0209(a), (f)).

What's the difference between LCMHC, LCSW and LMFT in North Carolina?▾

Each license comes from a separate board with its own hour totals and supervision math. The LCMHC track requires 3,000 hours, 2,000 direct, with at least 100 hours of supervision at a rate of one hour individual or two hours group per 40 hours of practice (21 NCAC 53 .0205(a), .0208). The LCSW track requires 3,000 paid clinical hours with supervision no less than once every two weeks and at least one hour per 30 hours worked (21 NCAC 63 .0211(a)(1), (a)(3)). The LMFT track requires 1,500 hours, including 500 relational hours, with a flat 200 hours of supervision and a one-hour monthly minimum (G.S. 90-270.54(a)(1)b; 21 NCAC 31 .0502(d), (e)).

How long can I hold an LCMHCA, LCSWA or LMFTA license?▾

The three associate licenses run on different clocks. The LCMHCA has no stated time limit and renews while hours are incomplete (21 NCAC 53 .0702(6)). The LCSWA is capped at six years total, across three renewal cycles (G.S. 90B-7(f)). The LMFTA runs three years from issuance and cannot be renewed, though a one-year extension is available by petition (G.S. 90-270.54A(b), (c)).

Does North Carolina require a jurisprudence exam for therapists?▾

Only for counselors. The North Carolina Board of Licensed Clinical Mental Health Counselors requires a no-fail jurisprudence exam for licensure application "at each level" and for every consecutive renewal period (21 NCAC 53 .0305(c)). The Social Work Certification and Licensure Board and the Marriage and Family Therapy Licensure Board do not require one.

How many supervision hours do I need in North Carolina?▾

It depends on the track. LCMHCA supervision totals at least 100 hours, with no less than three-quarters of it individual (21 NCAC 53 .0205(a)). LCSWA supervision also totals at least 100 hours, with a 25-hour cap on group supervision (21 NCAC 63 .0211(a)(3)). LMFTA supervision totals 200 hours, with a separate one-hour monthly minimum until the LMFT is issued (21 NCAC 31 .0502(d)).

Can an LCSWA be supervised by someone at the same practice?▾

Yes, as long as the supervisor is not the associate's own employee. The Position Statement on Clinical Supervision bars supervision by the associate's therapist, parent, spouse, former spouse, sibling, child, employee, household member, or anyone in a romantic, domestic or familial relationship with the associate, and adds that "a supervisor shall not be considered an employee of the associate licensee if the only compensation received by the supervisor is payment for actual supervisory hours" (Position Statement on Clinical Supervision, amended 05/01/26).

Requirements verified against 21 NCAC 53, 21 NCAC 63 and 21 NCAC 31 of the North Carolina Administrative Code and G.S. Chapter 90 Articles 24 and 18C and G.S. Chapter 90B, current as of September 2026. Statutes, rules and fees can change. Confirm current requirements at the NCBLCMHC, the NCSWCLB, or the NCMFTLB before making decisions about your licensure path.
